The Vancouver Primary Care Networks bring you an online series to connect team members working together in PCNs. The goals of the series are to learn, connect and share questions and experiences to help Interprofessional Team (IPT) Clinicians and Primary Care providers referring to the IPT to optimize how they work together to support their patients. To view a summary report from the first 5 sessions click here.
These events are open to family physicians, nurse practitioners and registered nurses currently referring to the IPT.
